Changing lanes on mine safety

Last year we attended a routine line up meeting at an underground mine. After a recent serious injury at this mine, safety managers had assessed and amended several key safety procedures with the intent of preventing similar incidents in the future. The changes had been disseminated to all shift supervisors, whose job it was to notify all crew members. The crew listened attentively as the shift supervisor outlined the changes to procedures. At the end of the meeting, attendance was logged to ensure all crew members received the updated information. The process was deemed to be complete. But something was missing.
